Dr. Green operates his practice under a membership model for all patients. Please review Dr. Green’s Membership prior to contacting our office as we have limited new patient openings.
Upon completion of this New Patient Intake Form, please mail, fax or drop it off at one of our offices.
If you are unable to print the form, please call us at (802) 238-8603 and we will gladly send it to you. Send completed forms via US mail, fax (855-886-6950), or email (admin@vtnaturalfamilymedicine.com), or you can easily drop it by our office during regular business hours—we'd love to meet you!

Steps to Becoming A Patient.
1) 15 minute Meet and Greet (optional & free of charge)
This is your chance to meet Dr. Green and see if you think you and he would be a good fit working together.
2) 45 minute Initial Visit Consultation
You will give Dr. Green a detailed history of your health and health issues, and he will give you an assessment of what he thinks may be going on, and what labs/imaging/referrals would be recommended. If at the end you want to continue care you will have the opportunity to sign up as a patient in his membership. (This visit is either billed to your insurance or you can pay out of pocket)
3) 45 minute Follow Up Visit
At this visit you will go over any results from labs/imaging, and/or recommendations from referrals. Then you & Dr. Green will discuss what Integrative & Holistic treatment options would be best for you as an individual.
Common Questions:
1) Do you see patients out of state?
Yes! We require you to have one in person visit per 12mo if we are going to prescribe any medication
2) How long is the membership for?
Our Membership is based on a 12mo commitment, but after the initial 3mo you can cancel at any time w/o a financial penalty.
3) What if I only want to pick Dr Green's brain about my issues, and I'm not sure if I want to be a member?
That's fine! You can can set up a meet and greet, or just sign up for an Initial Visit Consultation, and Dr. Green will let you pick his brain! It's up to you if you want to join the clinic after that visit.
4) Are you accepting new patients?
Currently we are! We have a limited number of membership slots available.
