An individualized treatment plan that combines the latest medical research with the most effective natural therapies available.


Before Your First Visit

For shorter waiting room times, please fill out the New Patient Form prior to your arrival. If you cannot print the form from home, please feel free to call us at (802) 238-8603 and we will gladly send it to you free of charge. Please send it to us via US Mail or FAX (802-732-9133), or you can easily drop it by our office during regular business hours - we'd love to meet you. 
Due to HIPAA regulations, we are unable to request an emailed form.
We must have your completed form in our office at least three business days before the appointment so that we may enter and review your health history and answer to questions.

First Visit

Plan on spending 45-75 minutes at your first visit (subsequent visits are 15-60 minutes.  We take time to understand you and your health in a comprehensive manner. At your visit, we will first obtain a total health history, then conduct a thorough physical examination, and assess your illness(es) including likely cause(s). This will allow us to create an individualized treatment plan that combines the latest medical research with the most effective natural therapies available. Therapies prescribed include, as appropriate: dietary nutritional, herbal medicine, nutraceuticals, pharmaceuticals, essential oils, and referrals to specialists.

Bring your insurance card. Please bring any recent lab work (within 6-12 months). 
Optional: Bringing a 3-10 day diet diary can be helpful but is not required.

INSURANCE COVERAGE:

We are proud to offer our patients insurance based naturopathic health care services.  We are in network with and provide coverage with all of the Vermont-based insurances.  We are listed as either primary care physicians or as specialists for insurance companies, depending on how we can best serve you (I.e if you need a primary care physician we are happy to be your PCP, but if you already have one you like we are happy to be your specialist and work alongside your PCP to help with your current issues). 

Our office policy is to collect for services at the time of the visit for all out-of-state based insurances and for any patient that has not yet met their deductible, and we encourage you to know (as it's your responsibility) if you have an unmet deductible.  Below is a list of common insurances patients have, with coverage information.

Vermont-based Insurances

Blue Cross Blue Shield of Vermont & Federal Blue Cross Blue Shield-We are in network with both of them, and can be your primary care physician or specialist.

CBA Blue-We are in network with them, and can be your primary care physician or specialist. 

Cigna of Vermont – We are in network with them, and can be your primary care physician or specialist. 

MVP Healthcare-We are in network with them, and can be your primary care physician or specialist.

Vermont Medicaid (aka Green Mountain Care/Dr. Dynasaur/etc)– We are in network with them, and can be your primary care physician or specialist.  We are very proud to say that currently at least 40% of our patients are Vermont-Medicaid patients..

Non-Vermont-based Insurances (i.e. Out of State based Plans)

Anthem BCBS– We frequently have patients request us to bill Anthem BCBS in case they might cover our services, in the case that they don't know if they are covered to see us or not.  Most often they cover us, although sometimes they don't.  We're happy to do so, and our office policy is to collect for services at the time of the visit until we have confirmation of coverage.

Blue Cross Blue Shield of any other state than Vermont-We recommend that you confirm that naturopathic medicine is covered by your out of state plan. We have found that most out of state BCBS plans cover our services, but unfortunately some do not. It is our policy to collect at time of service for all out of state BCBS plan until we can confirm coverage.

United – We frequently have patients request us to bill United in case they might cover our services, in the case that they don't know if they are covered to see us or not.  Sometimes they cover our services, sometimes they do not.  We're happy to do so, and our office policy is to collect for services at the time of the visit until we have confirmation of coverage.

Aetna- We frequently have patients request us to bill United in case they might cover our services, in the case that they don't know if they are covered to see us or not.  Sometimes they cover our services, sometimes they do not.  We're happy to do so, and our office policy is to collect for services at the time of the visit until we have confirmation of coverage.

Insurances not mentioned- We frequently have patients request us to bill their insurance company in case they might cover our services, in the case that they don't know if they are covered to see us or not.  We're happy to do so, and our office policy is to collect for services at the time of the visit  until we have confirmation of coverage
